Ahmimishäiriö ilman laihuushäiriötä ei vaikuttane luuntiheyteen.

Tutkimuksessa «Sundgot-Borgen J, Bahr R, Falch JA ym. Normal bone...»1 tutkittiin ruokavaliota ja syömishäiriöiden historiaa, kuukautishistoriaa, fyysisen aktiviteetin historiaa (haastattelu) ja luuntiheyttä (kaksoisröntgensäde-absorptiometrialla) 43 ahmimishäiriö-, 13 laihuushäiriö- ja 17 terveellä kontrollipotilaalla (ikä 18–29 vuotta). Kaikille laihuushäiriönaisille oli kehittynyt amenorreaa heidän oireiden alkamisesta lähtien, ja 68 %:lla ahmimishäiriöpotilaista oli kuukautishäiriöhistoria. Laihuushäiriöpotilailla oli matalampi luuntiheys kuin ahmimishäiriöpotilailla ja kontrolleilla kaikissa tutkituissa luissa. Säännöllinen liikunta oli yhteydessä luuntiheyteen ahmimishäiriöpotilailla, mutta ei laihuushäiriöpotilailla.

Toisessa tutkimuksessa «Davies KM, Pearson PH, Huseman CA ym. Reduced bone...»2 tutkittiin luuntiheyttä käsivarressa ja selkärangassa 26:lla laihuushäiriötä sairastavalla (ikä 20 ± 6 vuotta (SD)), 11:lla ahmimishäiriötä sairastavalla (ikä 26 ± 8 vuotta) ja 26:lla (ikä 25 ± 7 vuotta) potilaalla, joilla oli sekä laihuus- että ahmimishäiriön piirteitä. Mittaustuloksia verrattiin kontrolliryhmän tuloksiin.

Sekä laihuushäiriöpotilailla että laihuushäiriö/ahmimishäiriöpotilailla mittaustulokset olivat alentuneita kontrolleihin verrattuna, mutta ahmimishäiriöpotilaiden tulokset eivät erottuneet kontrollipotilaiden tuloksista.

In this Swedish cross-sectional study «Naessén S, Carlström K, Glant R ym. Bone mineral d...»3 77 bulimic patients (age 27.5 ± 5.3 years, BMI 22.0 (19.0–33.0 kg/m2) and 56 age- and body mass index (BMI)-matched healthy controls (age 28.6 ± 5.8 years, BMI 22.2 (19.0–29.5) kg/m2) were examined with respect to BMD (dual energy X-ray absorptiometry) and to serum levels of hormones and metabolic factors.

Bulimics had significantly lower spinal BMD (1.06± 0.14 vs. 1.11 ± 0.12 g/cm2, p<0.01) and higher frequency of osteopenia in the total body than controls (10/77, 13 % vs 1/56, 0.2 %, respectively, p<0.01). Furthermore, bulimic women had significantly lower levels of estradiol-17beta and free thyroxine and significantly higher cortisol levels compared with controls. Among the bulimics, 31.2 % had present menstrual disturbance, 51.9 % had a history of amenorrhea and 23.4 % had previous anorexia nervosa. Subgroups of bulimics with a history of amenorrhea and previous anorexia nervosa had significantly lower total total BMD (1.09 ± 0.08 vs 1.15 ± 0.07 g/cm2, P<0.001 and 1.09 ± 0.08 vs 1.15 ± 0.08 g/cm2, P<0.001 respectively) and spinal BMD than controls, whereas those without such history did not differ from the controls. In univariate analysis, a history of amenorrhea, cortisol, testosterone, previous anorexia nervosa, and BMI showed significant associations with spinal BMD. Multiple regression analysis including all significant variables revealed previous anorexia nervosa to be the strongest determinant of spinal BMD, accounting for 34 % of the variance, while associations between endocrine factors and BMI disappeared.

Low bone mass in bulimics may be explained by previous anorexia nervosa, whereas endocrine variables related to BMD seem to be secondary determinants that are dependent on previous anorexia nervosa and BMI.

Tutkimuksia nuoremmista potilaista ei ole, mutta on todennäköistä, että tulokset eivät heillä olisi samansuuntaisia.
