Obstruktiivisen uniapnean CPAP-hoito parantaa endoteelin toimintaa.

Endoteelitoiminta FMD-menetelmällä mitattuna on sydän- ja verisuonitapahtumien ja kuolemien riskin itsenäinen ennustetekijä «Shechter M, Issachar A, Marai I ym. Endoteelitoimintaa voidaan parantaa eri hoitomenetelmin, jolloin tavoitellaan kliinisesti merkittävän sydän- ja verisuonisairauden ehkäisyä. Obstruktiivisessa uniapneassa on todettu sekä endoteelitoiminnan heikkenemistä «Trzepizur W, Gagnadoux F, Abraham P ym. Microvascu...»5, «Noda A, Nakata S, Koike Y ym. Sleep apnea...»9 että lisääntynyttä sydän- ja verisuonisairastuvuuden riskiä. Siksi onkin kliinisesti mielekästä selvittää, voidaanko uniapnean CPAP-hoidolla parantaa endoteelitoimintaa ja sitä kautta vähentää sydän- ja verisuonisairastuvuuden riskiä.

Virtausvälitteisen vasodilataation (flow-mediated dilatation, FMD) mittausta pidetään luotettavimpana menetelmänä endoteelitoiminnan muutosten arvioimiseen «Corretti MC, Anderson TJ, Benjamin EJ ym. Muita menetelmiä ovat laskimo-okluusiopletysmografia (venous occulusion plethysmography, VOP), perifeerinen valtimotonometria (peripheral arterial tonometry, PAT), ihon laser-doppler virtausmittaus (skin laser doppler flowmetry) ja veren nitraatti/nitriittitasojen mittaus.

Meta-analyysissä «Schwarz EI, Puhan MA, Schlatzer C ym. Effect of CP...»12 vuosilta 2004–2013 tutkittiin 49 julkaisussa raportoitua CPAP-hoidon vaikutusta endoteelitoimintaan. Meta-analyysiin kelpuutettiin 8 RCT-tutkimusta, joissa yhteensä 245 potilaalla tutkittiin 2–24 viikon kestoisen CPAP-hoidon vaikutusta endoteelitoimintaan. Vaikutusta verrattiin uniapneapotilaisiin, joita seurattiin vastaava aika ilman hoitoa tai lume-CPAP-hoidon jälkeen.

Eff...»18CPAP-hoidon todettiin merkitsevästi parantavan heikentynyttä endoteelitoimintaa. 2 tutkimuksessa «Comondore VR, Cheema R, Fox J ym. Effects of ...»20 merkitsevää eroa ei voitu osoittaa. 4 tutkimuksessa FMD parani keskimäärin 3,87 % (95 % luottamusväli 1,93–5,80 %, p < 0,001). 1 tutkimuksessa endoteelifunktion paraneminen voitiin osoittaa myös VOP-menetelmällä, mutta PAT-menetelmällä eroa ei saatu. Kokonais-NOX lisääntyi merkitsevästi 1 tutkimuksessa 12 viikon onnistuneen CPAP-hoidon aikana.

Kommentti: 8 tasokkaasta RCT-tutkimuksesta 6 tutkimuksessa voitiin osoittaa tilastollisesti merkitsevä endoteelitoiminnan paraneminen CPAP-hoidon aikana. 2 muussa tutkimuksessa ero ei ollut merkitsevä. Suurin painoarvo oli parhaiten validoidulla FMD-menetelmällä (4 tutkimusta), mutta samansuuntainen tulos saavutettiin myös 2 muulla menetelmällä, joten löydös ei ollut menetelmäriippuvainen. RCT-tutkimusten tulokset ovat yhteneväiset myös lukuisampien muiden tutkimusten kanssa, joissa ei ollut käytetty RCT-asetelmaa. Vähentääkö CPAP-hoidon aikainen endoteelitoiminnan paraneminen käytännössä myös uniapneapotilaisen riskiä sairastua myöhemmin sydän- ja verisuonisairauteen, on toistaiseksi osoittamatta.
