Keskeinen sanoma

  • Sepelvaltimon äkillisestä ahtautumisesta tai tukkeutumisesta johtuvia oireistoja nimitetään sepelvaltimotautikohtauksiksi (ICD-10:n diagnoosiluokat I20.0, I21, I22).
    • Niihin kuuluvat
      • epästabiili angina pectoris
      • sydäninfarkti ilman ST-nousuja ja
      • ST-nousuinfarkti.
  • Epästabiilin angina pectoriksen ja sydäninfarktin ilman ST-nousuja hoitoperiaatteet ovat samat, kun taas ST-nousuinfarktin hoito on erilainen varsinkin akuuttivaiheessa.
  • Tässä suosituksessa sepelvaltimotautikohtauksella tarkoitetaan epästabiilia angina pectorista ja sydäninfarktia ilman ST-nousuja.
  • Sepelvaltimotautikohtauksen hoidossa on tarpeen arvioida yksilöllisesti, millaisessa vaarassa potilas on, ja valita sen perusteella hoitolinja.
    • Vaaran arviointi pohjautuu
      • oireisiin ja esitietoihin
      • kliiniseen tutkimukseen
      • EKG-muutoksiin ja
      • sydänlihasmerkkiainemäärityksiin.
    • Suuren vaaran potilaille tulee aloittaa tehokas antitromboottinen lääkitys, ja heidät pitää ohjata varhaiseen sepelvaltimoiden varjoainekuvaukseen.
    • Pienen vaaran potilaat voidaan yleensä kotiuttaa nopeasti, ja jatkoselvittelyt tehdään tällöin polikliinisesti. Jatkohoidossa on oleellista puuttua sepelvaltimotaudin vaaratekijöihin ja huolehtia potilaan neuvonnasta.

Tavoitteet

  • Suosituksen tavoitteena on tehostaa ja yhdenmukaistaa sepelvaltimotautikohtauksen saaneiden potilaiden vaaran arviointia ja kohtauksen hoitoa sekä vähentää näiden potilaiden kuolemia ja muita sydäntapahtumia.
  • Suositus on tarkoitettu perusterveydenhuollossa ja erikoissairaanhoidossa sepelvaltimotautikohtauspotilaita hoitaville lääkäreille ja muulle henkilökunnalle.

Akuuttivaiheen lääkehoito

  • Sepelvaltimotautikohtauksen hoidon tärkeimmät tavoitteet ovat
    • oireiden ja sydänlihasiskemian helpottaminen ja
    • haitallisten sydäntapahtumien (kuolema, sydäninfarkti, uusiutuva iskemia) estäminen.
  • Samalla on huolehdittava siitä, että hoidolla ei aiheuteta potilaalle uusia vaaroja, kuten munuaisten toiminnan häiriöitä tai verenvuoto-ongelmia.
  • Lääkehoidon kulmakivet
    • Anti-iskeeminen lääkitys
      • beetasalpaaja
      • nitraatti
      • (kalsiuminestäjä).
    • Antitromboottinen lääkitys
      • Ensi sijassa verihiutaleiden toimintaan vaikuttavat lääkkeet
        • asetyylisalisyylihappo
        • ADP-reseptorin estäjät (tienopyridiinit): klopidogreeli (ensisijainen), prasugreeli (ei vielä Suomen markkinoilla)
        • glykoproteiini IIb/IIIa -reseptorien salpaaja.
      • Ensi sijassa hyytymisjärjestelmään vaikuttavat lääkkeet
        • hepariinivalmisteet ja niiden sukulaiset;
          • pienimolekyylinen hepariini, kuten enoksapariini (ensisijaineni) ja daltepariini
          • synteettinen pentasakkaridi fondaparinuksi
          • fraktoimaton hepariini (standardihepariini)
        • suora trombiininestäjä bivalirudiini.

  • Liuotushoidosta ei ole osoitettu olevan hyötyä sepelvaltimotautikohtauksessa, ja siihen liittyvä lisääntynyt aivoverenvuodon vaara voi jopa huonontaa potilaiden ennustetta «Indications for fibrinolytic therapy in suspected acute myocardial infarction: collaborative overview of early mortality and major morbidity results from all randomised trials of more than 1000 patien»28.
  • Kotiutumisen jälkeistä lääkehoitoa käsitellään kohdassa Jatkohoito.

Anti-iskeeminen lääkehoito

  • Beetasalpaajalääkitys aloitetaan heti kaikille potilaille, ellei sille ole vasta-aiheita «Beetasalpaajalääkitys aloitetaan heti kaikille potilaille, ellei sille ole vasta-aiheita.»B.
    • Beetasalpaaja-annos säädetään sellaiseksi, että potilaan leposyketaajuus asettuu 50–60 lyöntiin minuutissa, kuitenkin niin, että jos potilaalla esiintyy merkittävää sydämen vajaatoimintaa beetasalpaus aloitetaan varovaisemmin ja tavoitesyke on 70–80/min.
  • Nitraatti on peruslääke, jolla hillitään akuuttia iskemiaa ja rintakipua, mutta sen ennustetta parantavasta vaikutuksesta ei ole näyttöä «Nitraatilla hillitään rintakipua sekä akuuttia iskemiaa, mutta sen ennustetta parantavasta vaikutuksesta ei ole näyttöä.»C.
  • Dihydropyridiiniryhmän kalsiuminestäjiä voidaan käyttää sepelvaltimotautipotilaiden rintakivun hoitoon beetasalpaajien ja nitraattien lisäksi. Sykettä hidastava kalsiuminestäjän (verapamiili ja diltiatseemi) käyttö voidaan aloittaa rintakipuoireen hoitoon vain sellaisille potilaille, jotka eivät siedä beetasalpaajaa. Potilaalla ei saa esiintyä sydämen systolista vajaatoimintaa eikä toisen tai kolmannen asteen eteiskammiojohtumisen häiriöitä.
    • Kalsiuminestäjiä voidaan myös harkita lisättäväksi hoitoon, jos potilaan verenpaine on kohonnut muusta lääkityksestä (beetasalpaaja ja ACE:n estäjä tai angiotensiinireseptorin salpaaja) huolimatta.

Jatkohoito

  • Jatkohoidolla tarkoitetaan tässä yhteydessä kotiutumisen jälkeistä hoitoa ja seurantaa.
  • Sepelvaltimotaudin vaaratekijöihin (rasva-aineenvaihdunnan häiriöt, kohonnut verenpaine, diabetes ja tupakointi) puututaan mahdollisimman varhain jo sairaalajakson aikana.
  • Koska kohtauksen uusiutumistodennäköisyys on suurin kotiutusta seuraavina viikkoina, on potilaalle annettava selkeät menettelyohjeet sepelvaltimotautikohtaukseen viittaavien oireiden uusiutumisen varalta.
  • Potilaiden neuvonta ja elintapaohjaus tulee aloittaa jo sairaalahoidon aikana, vaikka hoitojakso olisi lyhytkin.
  • Neuvontaa ja elintapaohjausta jatketaan avohoidossa sairaanhoitajan vastaanotolla.
  • Pitkäaikaisseuranta vastaa vakaan sepelvaltimotaudin sekundaaripreventiota.
  • Työkyvyn arvioinnissa käytetään yleisiä sepelvaltimotautipotilaan arviointiperusteita.
  • Myös potilaskohtainen kuntoutussuunnitelma on jatkohoidon keskeinen osa.

Seuranta

  • Jatkoseuranta toteutetaan yksilöllisesti.
  • Jälkitarkastuskäynnillä keskeisiä tarkastelukohteita ovat
    • potilaan vointi (mahdollisesti tehdyn toimenpiteen tuloksen arviointi)
    • sepelvaltimotaudin vaaratekijät ja niiden hoito
    • lääkitys
    • työkyky ja
    • kuntoutustarve.
  • Depressio on pyrittävä havaitsemaan ja hoitamaan.
  • Jos oireet uusiutuvat pallolaajennusta seuraavien kuukausien aikana, on sepelvaltimoiden varjoainekuvaus yleensä aiheellinen eikä kliinistä kuormituskoetta useimmiten tarvita.
