Mikään yksittäinen yksilöterapiamuoto ei ilmeisesti ole muita parempi aikuisten laihuushäiriöpotilaiden avohoidossa.

In a randomized controlled trial «Zipfel S, Wild B, Groß G ym. Focal psychodynamic t...»1 of 242 adult women with anorexia nervosa (DSM-IV criteria or one diagnostic criterion absent and BMI of 15-18,5 kg/m2); exclusion criteria current substance abuse, use of neuroleptic drugs, psychotic or bipolar disorder, serious unstable medical problems and ongoing psychotherapy; from 10 German centers were recruited. They were randomized: 80 to focal psychodynamic therapy (DT), 80 to enhanced cognitive behaviour therapy (CBT), and 82 to optimized treatment as usual (UT). Treatment lasted 10 months and was conducted on weekly bases, standardized treatment manuals were used in DT and CBT.

Outcome measures were BMI, self assessment with eating disorder inventory-2 (EDI-2), structured clinical interview (SCID-I), full structured interview for anorexic and bulimic syndromes (SIAB-X), masked assessors’ assessment of the psychiatric status.

Results: At the end of treatment, 54 patients (22 %) were lost to follow-up, and at 12-month follow-up a total of 73 (30 %) had dropped out. At the end of treatment the mean gain in BMI was 0.73 kg/m² in DT group, 0.93 kg/m² in CBT group and 0.69 with optimized treatment as usual; no difference was noted between groups in weight gain (mean difference between focal psychodynamic therapy and enhanced cognitive behaviour therapy -0.45, 95 % CI -0.96 to 0.07; focal psychodynamic therapy vs optimised treatment as usual -0.14, -0.68 to 0.39; enhanced cognitive behave our therapy vs optimised treatment as usual -0.30, -0.22 to 0.83). At 12-month follow-up, the mean gain in BMI had risen further (1.64 kg/m², 1.30 kg/m², and 1.22 kg/m², respectively), but no differences between groups were recorded (0.10, -0.56 to 0.76; 0.25, -0.45 to 0.95; 0.15, -0.54 to 0.83, respectively).

With respect to the global outcome measure, patients allocated to focal psychodynamic therapy (DT) had higher recovery rates compared with those assigned optimized treatment as usual (UT) at 12-month follow-up. When applying a strict definition of recovery (a combination of objective measure [weight] and eating disorder pathology based on expert assessment), recovery rates were 7–13 % at 6 months and 14–19 % at 12 months, with no differences with respect to recovery rates between the treatments.

56 naista satunnaistettiin «McIntosh VV, Jordan J, Carter FA ym. Three psychot...»2 kolmeen hoitoon: 16 supportiiviseen kliiniseen hoitoon (specialist supportive clinical management, SSCM), 19 kognitiiviseen psykoterapiaan (CBT) ja 21 interpersoonalliseen terapiaan (IPT). Vastaavasti 11 (SSCM), 12 (CBT) ja 12 (IPT) potilaista jatkoi hoidon loppuun (ainakin 15 terapiakertaa jota annettiin 20 x 1 tuntia vähintään 20 viikon aikana). Seurannassa (n = 40) arvioitiin fyysinen tila, ruumiinkuva, itsearviointi ja strukturoitu kliininen haastattelu (sokkoutetusti). Arviointi sisälsi Hamiltonin depressiolomakkeen, Eating Disorder Examination -skaalan ja yleisen psykososiaalisen toimintakyvyn arvioinnin (GAF).

Terapian jälkeen toteutetussa arvioinnissa ei ryhmien välillä todettu eroja fyysisissä mittauksissa, kuten painossa, BMI:ssa ja ruumiin rasvakudoksessa. GAF oli parempi kliinisen hoidon ryhmässä verrattuna molempiin psykoterapiaryhmiin, jotka eivät eronneet merkitsevästi toisistaan.

Välittömästi hoidon jälkeen tapahtuneessa seurannassa (N = 43) 75 %:lla SSCM-ryhmän, 33 %:lla KBT-ryhmän ja 15 %:lla IPT-ryhmän potilaista oli hyvä hoitotulos. Viisi vuotta hoidon jälkeen tapahtuneessa seurannassa hyvä hoitotulos oli vastaavasti 42 %:lla SSCM-ryhmästä, 41 %:lla KBT-ryhmästä ja 64 %:lla IPT-ryhmästä «Carter FA, Jordan J, McIntosh VV ym.
