Anamnestiset oireet

• Nivelkipu
  • Kipu on jomottavaa, pahenee liikkuessa ja lievittyy levossa, mutta sairauden edetessä se voi muuttua jatkuvaksi ja vaivata myös yöllä «O'Reilly S, Doherty M. Signs, symptoms and laboratory tests.   • Polven nivelrikon aiheuttama kipu on varsin paikallista. Lonkkanivelperäinen kipu tuntuu usein maksimaalisena reiden etupinnalla aivan nivustaipeen alapuolella, mutta se voi olla laaja-alaisempi ja epätarkempi ja säteillä reiden etu-, ulko- ja takapinnalle «O'Reilly S, Doherty M. Kliininen tutkimus

• Inspektiolla arvioidaan liikkumista tutkimustilanteessa, nivelen asentoa seistessä ja ulkomuodon muutoksia.
  • Potilasta pyydetään kyykistymään ja kävelemään tutkimushuoneessa, ja samalla tarkastellaan kävelyä eri suunnista.

• Manuaalinen tutkiminen
  • Polven ja lonkan liikelaajuudet tutkitaan goniometrilla «Solonen KA, Nummi J. Nivelten liikkeiden mittaaminen. Suom Lääkäril 1971;20 (eripainos)»19. Lonkkanivelestä tutkitaan passiivinen sisään- ja uloskierto, koukistus ja ojennus ja polvinivelestä koukistus- ja ojennusliikelaajuus.
    • Lonkkanivelrikossa lonkan sisäänkierto vaikeutuu herkimmin.

  • Polven osalta tehdään tarkempi erotusdiagnostinen manuaalinen tutkimus, jossa arvioidaan nivelensisäinen nestemäärä, nivelraon aristus, nivelen stabiliteetti sekä ristiside- ja meniskivaurioiden mahdollisuus.

Laboratoriolöydökset

• Verikokeilla ja nivelnesteestä tai virtsasta määritetyillä rusto- ja luukudoksen biokemiallisilla merkkiaineilla ei ole merkitystä nivelrikon kliinisessä diagnostiikassa eikä seurannassa «O'Reilly S, Doherty M. Kuvantamistutkimukset

• Polven ja lonkan röntgenkuvaus ja luokitus
  • Polven etukuva otetaan lievässä fleksiossa taka-etusuunnassa (PA-kuva) seisten, molemmat polvet samaan kuvaan «Mazzuca SA, Brandt KD, Buckwalter KA ym. Field test of the reproducibility of the semiflexed metatarsophalangeal view in repeated radiographic examinations of subjects with osteoarthritis of the knee.»22 (kuva «Polven etukuva otetaan lievässä fleksiossa taka-etusuunnassa (PA-kuva) seisten, molemmat polvet samaan kuvaan»1
    
    ). Lievässä fleksiossa seisten otettu polven etusuunnan röntgenkuva voi näyttää nivelraon kaventumisen aiemmin kuin suorin polvin otettu kuva «Lievässä fleksiossa seisten otettu polven etusuunnan röntgenkuva voi näyttää nivelraon kaventumisen aiemmin kuin suorin polvin otettu kuva.»C.
    • Putken 10 asteen kallistuksella röntgensäteet saadaan sääriluun nivelpinnan suuntaan ja nivelraon leveys luotettavammin arvioitua.
  • Sivukuva otetaan joko seisten tai makuulla.
  • Jos patellofemoraalinivelen tilannetta tarvitsee selvittää tarkemmin kuin mitä sivukuvasta on arvioitavissa, käytetään ns. skyline-projektiota, jolloin kuva otetaan ylä- ala suunnassa polvi 30 asteen fleksiossa «Jones AC, Ledingham J, McAlindon T ym.   • Lonkkien röntgenkuvat otetaan seisten tai makuulla etu-takasuunnassa (AP). Maaten kuvattaessa saadaan parempi kuvanlaatu etenkin lihavilla potilailla. Seisten otetuissa kuvissa nivelraon leveys ei muutu merkittävästi maaten otettuihin kuviin verrattuna «Auleley GR, Duche A, Drape JL ym.     • Lonkan, lantiokaaren ja alaraajojen pituuseron arvioinnissa seisten otettu lantiokuva voi olla tarpeen.
    • Erotusdiagnostiikkaa ajatellen kuvausta voidaan täydentää lonkan sivukuvalla tai Lauensteinin projektiolla.
  • Työryhmä suosittaa käytettäväksi Kellgrenin ja Lawrencen luokitusta polvi- (taulukko «Polvinivelrikon röntgenkuvaan perustuva radiologinen Kellgrenin ja Lawrencen luokitus .»4) ja lonkkanivelrikon (taulukko «Lonkkanivelrikon röntgenkuvaan perustuva radiologinen Kellgrenin ja Lawrencen luokitus .»3) radiologisen vaikeusasteen luokittelussa. Luokituksen toistettavuus näyttää olevan vähintään kohtalainen «Lonkka- ja polvinivelrikon Kellgren amp; Lawrence -luokituksen toistettavuus näyttää olevan vähintään kohtalainen.»B. Suomessa polvinivelrikon yhteydessä yleisesti käytetyn Ahlbäckin luokituksen toistettavuus on osoittautunut huonommaksi «Galli M, De Santis V, Tafuro L. • Polven ja lonkan magneettikuvaus
  • Magneettikuvaus on ensisijainen täydentävä kuvantamismenetelmä, tietokonetomografiaa ei tarvita artroosin diagnostiikassa.
  • Jos röntgenkuvien perusteella epäillään avaskulaarista nekroosia, pahanlaatuista kasvainta tai infektiota, magneettikuvaus on suositeltava.
  • Magneettikuvaus näyttää rustomuutokset varhaisemmassa vaiheessa kuin röntgenkuvaus, mutta validoituja artroosin luokitus- tai seurantamenetelmiä ei vielä ole «Hayes CW, Jamadar DA, Welch GW ym.   • Polviartroosissa röntgenkuvista arvioitavan Kellgrenin ja Lawrencen luokan on todettu korreloivan magneettikuvauksessa näkyviin rustopuutoksiin, osteofyytteihin ja nivelnestemäärän lisäykseen «Hayes CW, Jamadar DA, Welch GW ym. Osteoarthritis of the knee: comparison of MR imaging findings with radiographic severity measurements and pain in middle-aged women. Radiology 2005;237(3):998-1007. »26.
  • Ruston normaali paksuus vaihtelee eri nivelpinnoilla ja henkilöillä «Adam C, Eckstein F, Milz S ym. • Erotusdiagnostisesti hälyttävät, yleensä akuutisti tai subakuutisti ilmaantuneet niveloireet vaativat lisätutkimuksia, jolloin muun muassa nivelnesteen analyysi, muut laboratoriokokeet ja mahdollisesti röntgentutkimukset ovat aiheellisia «O'Reilly S, Doherty M. Diagnostiset määritelmät

• Nivelrikko voidaan jakaa kolmeen pääryhmään diagnostiikassa käytettyjen menetelmien mukaan:
  • Radiologinen nivelrikko. Diagnoosi perustuu pelkästään radiologisiin löydöksiin, kuten arvioon osteofyyttien esiintymisestä tai nivelraon kaventumisesta tai molemmista ja arvioon muista nivelen luumuutoksista.
    • Radiologisen nivelrikon rajana on useimmissa tutkimuksissa käytetty Kellgrenin ja Lawrencen luokkaa 2.
  • Kliininen nivelrikko. Kliininen nivelrikkodiagnoosi perustuu potilaan kuvaamiin oireisiin, statuslöydöksiin, aiempiin potilasasiakirjoihin ja mahdollisesti tehtyihin laboratoriotutkimuksiin.
  • Kliinis-radiologinen nivelrikko. Diagnoosiin vaaditaan radiologisten löydösten lisäksi tyypilliset nivelrikon oireet, joita ovat esimerkiksi nivelkipu ja -jäykkyys, ja tarvittaessa erotusdiagnostiset laboratoriotutkimukset.
• Tieteellisessä kirjallisuudessa on esitetty monia kriteerejä lonkka- ja polvinivelrikolle, mutta yksiselitteistä ja yleisesti hyväksyttyä diagnoosikriteeristöä ei ole olemassa «Roos E. Kliniska kriterier bästa stöd för diagnosen lätt till måttlig artros. Läkartidningen 2002;99:4362-4»31.
