Tupakoinnin vaikutus hoitomenetelmään

  • Tupakoivilla kantaluun murtumapotilailla ja akillesjänteen repeämäpotilailla on huomattavasti kohonnut leikkaushoidon komplikaatioiden riski. Tupakointi, kuten muutkin leikkaushoidon riskitekijät, otetaan huomioon päätettäessä, hoidetaanko vamma leikkauksella vai konservatiivisesti.
  • Kiireettömiä jalkateräleikkauksia ja kiireettömiä alaselän luudutusleikkauksia tehdään useissa sairaaloissa pääsääntöisesti vain, jos potilas lopettaa tupakoinnin tai on ollut vähintään kaksi kuukautta tupakoimatta, jotta hoitotulos on mahdollisimman hyvä.
  • Kaikki potilaat hoidetaan kuitenkin niin hyvin kuin mahdollista. Potilasta ei jätetä leikkaushoidon ulkopuolelle, vaikka hän ei lopettaisi tupakointia. Komplikaatioiden riskitekijät arvioidaan. Jos leikkaukseen liittyvien vakavien komplikaatioiden riski on korkea, arvioidaan tapauskohtaisesti, mikä on keskimäärin leikkaushoidon tulos ja toisaalta konservatiivisen hoidon tulos, jos konservatiivinen hoito on mahdollinen. Päätetään, hoidetaanko potilas leikkauksella vai konservatiivisesti. Lääketieteellisen arvion perusteet käydään läpi potilaan kanssa ja kirjataan sairauskertomukseen. Päivystyspotilaiden ja kiireellisten leikkausten päätöksiin potilaan mahdollinen tupakointi ei vaikuta.
  • Kuitenkin kaikille leikkauspotilaille suositellaan tupakoinnin lopettamista viimeistään leikkauksesta päätettäessä.
