a. Hammaskaari, josta ei puutu hampaita

b. Toispuoleisesti lyhentynyt hammaskaari, jossa purentaparit puuttuvat vain toiselta puolen hammaskaarta

c. Molemmin puolin lyhentynyt hammaskaari, jossa leukaneljänneksestä puuttuvat vähintään molaarit ja enintään premolaarit ja molaarit

d. Voimakkaasti lyhentynyt hammaskaari, jossa on jäljellä hampaat vain kulmahampaasta kulmahampaaseen

Tiivistelmä ja potilasversio

Suosituksen tavoitteet ja kohderyhmät

  • Suosituksen tavoitteena on antaa ohjeita siihen,
    • miten voidaan pitää yllä riittävää purentakykyä tai palauttaa se ja ehkäistä haitalliset purentamuutokset potilailla, joilla on lyhentynyt tai lyhentymisvaarassa oleva hammaskaari
    • milloin olemassa oleva hampaisto kannattaa kuntouttaa niin, ettei lyhentynyttä hammaskaarta pidennetä proteettisesti
    • milloin ja miten puuttuvia hampaita korvataan lyhentyneessä hammaskaaressa.
  • Suositus ei ota kantaa
    • täydelliseen hampaattomuuteen
    • hammaskaaren puutosaukkoihin
    • kasvo- ja leukavammojen aiheuttamiin hammaspuutoksiin
    • synnynnäisiin kehityshäiriöihin
    • muista syistä kuin hammassairauksista johtuvaan hammaspuutokseen.
  • Suosituksen kohderyhmänä ovat erityisesti suun terveydenhuollon ammattilaiset.

Esiintyvyys

Tutkimus ja diagnostiikka

  • Anamneesi ja systemaattinen purentaelimistön kliininen tutkimus, jota on täydennetty kuvantamistutkimuksilla ja kipsimallisuunnittelulla, ovat olennaisia hoitosuunnitelman tekemisen ja hoidon ennusteen kannalta (ks. kuva «Potilaan hoitokaavio»2).
  • Hoitosuunnitelman tekemistä edeltää potilaan ja hammaslääkärin välinen keskustelu (dialogi), jossa otetaan huomioon potilaan subjektiiviset sekä hammaslääketieteelliset ja lääketieteelliset näkökohdat (ks. kuva «Potilaan hoitokaavio»2).

Potilaan subjektiiviset näkökohdat

  • Hammaslääketieteellisten aiheiden lisäksi suunnittelun lähtökohtana on potilaan subjektiivinen hoidon tarve.
  • Ulkonäköön liittyvät seikat voivat olla potilaalle tärkeämpiä kuin toiminnalliset seikat.
  • Lyhentyneen hammaskaaren hoidon suunnittelun tulee olla yksilöllistä ja ongelmakeskeistä.
  • Hampaiden puutos heikentää suun terveyteen liittyvää elämänlaatua «Hampaiden puutos heikentää suun terveyteen liittyvää elämänlaatua.»A.
    • Elämänlaatua heikentäviä tekijöitä ovat pureskeluun liittyvät ongelmat sekä sosiaaliset ja ulkonäköön liittyvät ongelmat.
    • Subjektiivisesti etuhampaat ja premolaarit koetaan tärkeiksi «Elias AC, Sheiham A. The relationship between sati...»9 mutta takahammasalueen puutosta ei pidetä niinkään merkittävänä.
