Hur kan man hänvisa till God medicinsk praxis-rekommendationen? «K1»1

Den här svenskspråkiga God medicinsk praxis-rekommendationen har översatts från den finska God medicinsk praxis-rekommendationen (Käypä hoito -suositus Lonkkamurtuma «Lonkkamurtuma»1). Om det finns skillnader i texterna gäller den uppdaterade finskspråkiga versionen.

Huvudsakligen finns evidenssammandragen och bakgrundsmaterialen samt internetlänkarna på finska.

Centrala rekommendationer

  • Höftfraktur är en allvarlig fallskada då den drabbar äldre personer. Den leder ofta till nedsatt rörelse- och funktionsförmåga och dödligheten hos höftfrakturpatienter är hög. För samhället utgör höftfrakturerna de mest betydelsefulla sjukdomarna som leder till hjälpbehov och långvård. I Finland opereras årligen cirka 6 000 höftfrakturer.
  • Risken för höftfraktur kan minskas genom reducerad inverkan av faktorerna som ökar fallrisken och försämrar benhållfastheten.
    • Risken för fall hos äldre personer måste identifieras. Detta gäller särskilt viktigt för äldre personer som har fallit tidigare. Deras situation ska bedömas ingående och faktorerna som ökar fallrisken bör åtgärdas.
  • Effektiv behandling av höftfrakturpatientens smärta skapar en god grund för mobilisering och bidrar till att förebygga komplikationer. Smärtbehandlingen omfattar farmakologisk smärtlindring och nervblockader.
  • En höftfrakturoperation kan utföras under narkos eller spinalbedövning.
  • Operationen utförs utan dröjsmål enligt en metod som tillåter patienten att stöda på den opererade extremiteten med full vikt efter operationen.
  • Patientens mobiliseras genast efter operationen. Den postoperativa fysiska träningen ska också innehålla progressiv muskelstyrketräning.
  • Prognosen för höftfrakturpatienter kan förbättras och vårdkostnaderna minskas då vården och rehabiliteringen av höftfrakturpatienter koncentreras till enheter insatta i geriatriska sjukdomar. Även höftfrakturpatienter med minnessjukdom har nytta av multiprofessionell geriatrisk rehabilitering.
  • Efter en höftfraktur måste nya benbrott förebyggas aktivt. Det viktigaste är att utreda orsakerna till den första höftfrakturen och att åtgärda omständigheterna som ökar fallrisken. Dessutom ska patienten ha ett tillräckligt intag av kalcium och D-vitamin, och läkemedelsbehandling av osteoporos bör övervägas.
  • Arbetet för höftfrakturpatienternas väl är multiprofessionellt och förutsätter därför smidigt samarbete och informationsutbyte mellan olika yrkesutbildade personer. Rehabiliteringen planeras utifrån patientens förutsättningar och målsättningen för rehabiliteringen. Patientens anhöriga tas aktivt med i rehabiliteringen.

Avgränsning av ämnet

  • Rekommendationen gäller frakturer på övre delen av lårbenet hos vuxna till följd av att de fallit omkull eller fallit från låg höjd.

Mål

  • Syftet med rekommendationen är att främja preventionen och behandlingen av höftfrakturer på ett omfattande och övergripande sätt samt att främja återhämtningen av patienternas dagliga funktioner.

Målgrupp

  • Rekommendationen är avsedd för alla yrkesutbildade personer och beslutsfattare som deltar i vården av höftfrakturpatienter inom social- och hälsovården.

Analys av riskfaktorerna för fraktur

Hur förebygga första höftfrakturen

  • Grunden för prevention av höftfrakturer är identifiering och god behandling av bakomliggande sjukdomar, mångsidig kost, fysisk aktivitet, förebyggande av fallolyckor samt förebyggande och behandling av osteoporos.
    • Särskild uppmärksamhet ska fästas vid personer som tidigare haft frakturer. Bland de tidigare frakturerna ska också kotfrakturer beaktas. De kan uppkomma med mycket lindriga symtom eller diagnostiseras slumpmässigt (God medicinsk praxis-rekommendationen Osteoporoosi «Osteoporoosi»2 (på finska); Osteoporos «Osteoporos»3 (på svenska)).
  • För att förebygga höftfrakturer behövs mångsidiga åtgärder på populationsnivå. Dessa åtgärder ska rikta sig mot den äldre befolkningen och personer som löper särskild risk för fraktur (tabell «Faktorer som ska beaktas vid förebyggande av höftfraktur...»4).

Diagnos

Klassificering av frakturer på övre delen av lårbenet

  • Indelningen och klassificeringen av frakturer i övre delen av lårbenet visas i bild «Indelning och klassificering av frakturer i övre delen av lårbenet (ICD 10)»1.
  • Frakturer på lårbenshalsen indelas i icke-dislokerade och dislokerade frakturer.
  • Frakturlinjen eller -linjerna vid pertrokantära frakturer går via utskotten eller mellan dem, men ovanför den nedre kanten av trochanter minor.
    • Den mest betydande av de otaliga klassificeringarna av pertrokantära frakturer är indelningen i grovt i stabila och instabila frakturer.
    • I den vanligaste AO/ASIF-klassifikationen motsvaras stabil fraktur av huvudklass A1 (med kvarstående medialt stöd).
    • Instabila pertrokantära frakturer motsvaras av huvudklasserna A2 (fraktur där medialt och dorsalt stöd saknas) och A3(transtrokantär fraktur där frakturlinjen sträcker sig till laterala cortex, eller omvänd pertrokantär fraktur) (bild «Trokanteeriset murtumat AO-luokituksen mukaan»2).
  • I subtrokantära frakturer går frakturlinjerna nedanför trokanterutskotten från trochanter minor högst 5 cm distalt.

Ordnande av vård och rehabilitering för höftfrakturpatienter

  • God vård för höftfrakturpatienter ska vara omfattande och multiprofessionell. Målet är
    • att återställa patientens rörelseförmåga och funktionsförmåga
    • att undvika komplikationer
    • att undvika ytterligare fall och frakturer.
  • Enligt tjänsteutbudsrådet för hälso- och sjukvården (PALKO) hör multiprofessionell, aktiverande och målinriktad rehabilitering efter höftfraktur till det offentligt finansierade tjänsteutbudet inom hälso- och sjukvården i Finland (internetlänk: Tjänsteutbudsrådet «http://palveluvalikoima.fi/lonkkamurtumakuntoutus»11).
  • Vården och rehabiliteringen av höftfrakturpatienterna ska koncentreras till kompetenta multiprofessionella centra med tillräckliga resurser.
  • Ortogeriatrisk behandling (d.v.s. behandling av ortopediska patienter där en geriatriker är med i vårdteamet från och med den akuta fasen) kan, i huvudsak på basis av observationsstudier, minska dödligheten och förkorta behandlingstiderna också utanför multiprofessionella geriatriska rehabiliteringsenheter «Grigoryan KV, Javedan H, Rudolph JL.   • När man väljer vårdprincip för höftfrakturpatienter måste patientens helhetssituation beaktas, eftersom olika patientgrupper har olika behov (bild «Behandlingschema för höftfrakturpatient»3, «gvr00074a.pdf»1). Sådana patientgrupper är
    • patienter som bott hemma före frakturen
    • äldre personer inom långvården.
  • Man har inte på ett övertygande sätt kunnat påvisa att patienter som vårdats dygnet runt före höftfrakturen skulle ha nytta av centraliserad intensiv rehabilitering «Muistisairaat lonkkamurtumapotilaat hyötyvät leikkauksenjälkeisestä moniammatillisesta geriatrisesta kuntoutuksesta.»A. Arbetsgruppen rekommenderar att dessa patienter återvänder till sin tidigare vårdplats så snart som möjligt. På vårdplatsen mobiliseras patienterna genast och man eftersträvar att återställa deras tidigare funktionsförmåga med hjälp av vårdarbete som stöder rehabiliteringen.
    • Man måste dock säkerställa att vårdplatsen har tillräcklig kompetens och beredskap för postoperativ vård, för identifiering av komplikationer, för administration av tillräcklig analgesi «Killington M, Walker R, Crotty M. The chaotic jour...»131 och att det finns tillräckliga fysioterapiresurser.
  • Oberoende av utgångsläget ska vården av höftfrakturpatienter vara väl planerad och påbörjas utan dröjsmål. God vårdkontinuitet är viktig då patienten flyttas från en vårdplats till en annan och inom vårdenheterna.
  • Det förekommer stor variation i ordnandet av vården av höftfrakturpatienter mellan sjukvårdsdistrikten i Finland, bland annat i fråga om hur länge patienten vårdas vid den opererande enheten. Undersökningar där man jämför olika vårdkedjor har dock inte gjorts.
    • Tidig överflyttning till enheten för den fortsatta vården verkar vara möjlig om patientens tillstånd är stabilt. Detta förutsätter dock tillräcklig beredskap vid enheten som ansvarar för den fortsatta vården och jour dygnet runt.
    • Enligt en undersökning baserad på riksomfattande vårdregister i Sverige var en total vårdtid på mindre än 10 dygn associerad med en ökad risk för dödsfall inom 30 dygn från utskrivningen «Nordström P, Gustafson Y, Michaëlsson K ym. Length...»132.
  • Det som är viktigt med tanke på slutresultatet för patienten är hur väl höftfrakturpatienternas vårdkedja fungerar och hurdana resultaten är och detta ska bedömas regelbundet. Vårdkedjan utvecklas kontinuerligt på basis av utvärderingarna.

Preopreativ bedömning av höftfrakturpatienterna

  • När patienten anlänt till sjukhuset ska hens fysiska, psykiska och sociala funktionsförmåga före höftfrakturen uppskattas och bedömas.     • Bland de kroniska sjukdomarna är KOL, diabetes, hjärtsvikt, rytmstörningar i hjärtat och myokardischemi de viktigaste med tanke på anestesi.
      • Om operationen måste skjutas upp grund av kroniska sjukdomar, ökar patientens mortalitet 2,5-faldigt inom en månad «Moran CG, Wenn RT, Sikand M ym. Early mortality af...»135.
      • För lungornas del bedöms risken för akut lunginfektion och för personer med kronisk lungsjukdom bedöms huruvida exacerbation föreligger.
    • Patientens läkemedel utreds omsorgsfullt, både dem som hen tar regelbundet och dem som hen tar vid behov. Orala diabetesläkemedel och alltför kraftiga blodtryckssänkande läkemedel läggs på paus.
    • Användningen av läkemedel som påverkar blodkoagulationen utreds och registreras.
      • Om INR-värdet hos en patient på warfarinbehandling är högt, är det inte värt att uppskjuta operationen, utan i stället justeras antikoagulationsnivån till en trygg nivå antingen med K-vitamin eller med PCC-preparat «Purmonen T, Törmälehto S, Säävuori N ym. Budget im...»136.
      • Enligt observationsstudier utgör användningen av klopidogrel ingen grund för att skjuta upp operationen eller att avstå från spinalbedövning «Doleman B, Moppett IK. Is early hip fracture surge...»137.
      • Forskningsdata saknas gällande behandlingen av höftfrakturpatienter som använder direkta antikoagulantia. För dessa patienter bedöms möjligheten att häva antikoagulationen eller att utföra operationen under narkos.
  • När patienten anlänt till sjukhuset bedöms också hens näringstillstånd, vätskebalans, smärtor, kroppstemperatur, urinblåsans och tarmens funktion, användningen av alkohol och tobak samt risken för trycksår.
  • En höftfrakturoperation kan vara befogad som palliativ åtgärd mot smärta för många patienter som annars inte skulle anses lämpliga för kirurgi.
  • Preoperativt är det förnuftigt att göra endast de undersökningar som påverkar vårdbeslutet eller som leder till åtgärder som förbättrar patientens operabilitet.
  • Före operationen
    • inleds omedelbart åtgärder för att korrigera störningar i elektrolytbalansen, hypovolemi och betydande anemi
    • Allvarliga organstörningar identifieras (t.ex. njursvikt, lunginflammation eller akut hjärtinfarkt) och behandlingen av dem inleds
      • Orsaken till patientens olycksfall kan ligga i någon akut sjukdom som måste åtgärdas i brådskande ordning.
    • Om patienten har hjärtsvikt, lungsjukdom eller diabetes i dålig balans korrigeras dessa i mån av möjlighet.
    • Excessiv antikoagulation hävs.
  • Om patienten inte har antikoagulationsbehandling och operationen inte kan utföras inom de följande 12 timmarna, inleds ventrombosprofylax med lågmolekylärt heparin «Leer-Salvesen S, Dybvik E, Dahl OE ym. Postoperati...»145.
  • Alla patienters hälar och bäcken skyddas med mjukt underlag för att undvika trycksår.
    • Om risken för trycksår anses vara förhöjd, används en madrass som förhindrar trycksår.
  • Under väntetiden före operationen ska patientens kroppstemperatur, smärtlindring samt vätske- och näringsintag uppföljas och justeras.

Smärta orsakad av akut fraktur och operationssmärta

  • Smärtbehandlingen genomförs enligt principerna för multimodal smärtbehandling (God medicinsk praxis-rekommendationen Kipu «Kipu»6 (på finska); Smärta «Smärta»7 (på svenska)) och påbörjas genast då frakturen uppstått.
  • Smärtbehandlingen bygger på paracetamol «Cuvillon P, Ripart J, Debureaux S ym. [Analgesia a...»160, «Bollinger AJ, Butler PD, Nies MS ym. Is Scheduled ...»161. För behandling av medelsvår och svår smärta används dessutom ledningsbedövning (nervblockad) och vid behov opioidanalgetika.
  • Med hjälp av ledningsbedövning (nervblockad) kan man minska patientens upplevda smärta och behovet av opioidanalgetika «Johtopuudutuksen avulla voidaan vähentää potilaan kokemaa kipua ja opioidin tarvetta.»A.
    • Patienten bör anläggas fascia iliaca- eller femoralisblockad genast vid ankomst till jouren «Niskanen RO, Strandberg N. Bedside femoral block p...»163.
      • Båda nervblockaderna är tekniskt enkla att lära sig och behovet av instrument är litet.
      • Ultraljudsstyrning underlättar anläggandet av nervblockaden.
    • Det är skäl att förnya nervblockaden före operationen, då patienten ligger på uppvakningsavdelningen och då hen ligger på vårdavdelningen, om tillfredsställande smärtlindring inte uppnås med regelbunden administration av paracetamol och måttliga doser av opioidanalgetika.
  • Paracetamol administreras initialt intravenöst. Efter operationen administreras paracetamol och andra smärtstillande läkemedel genom munnen genast då patienten kan ta vätska och mat genom munnen.
    • Engångsdosen av paracetamol är 1 g (500 mg om patienten väger mindre än 50 kg). Initialt administreras paracetamol till patienter i gott skick 4 gånger per dygn och till ålderstigna och till undernärda patienter samt till patienter med svår njursvikt 3 gånger per dygn.
    • Efter vårdperioden på sjukhus fortsätter smärtbehandlingen med paracetamol för patienter som fortfarande har smärta. Inom långvården är dygnsdosen dock 2 g på grund av leverskador och ökad blödningsrisk.
  • Trots att icke-steroida antiinflammatoriska medel (NSAID) är effektiva analgetika för frakturpatienter «Godoy Monzón D, Vazquez J, Jauregui JR ym. Pain tr...»164 är de ofta kontraindicerade för äldre patienter.
    • NSAID ska alltid användas med minsta effektiva dos och kortaste nödvändiga tid (rekommendationen God medicinsk praxis Kipu «Kipu»6 (på finska); Smärta «Smärta»7 (på svenska)).
  • Om tillräcklig smärtlindring inte uppnås med paracetamol, NSAID och nervblockad, administreras opioidianalgetika.
    • I Finland är erfarenheten av användningen av oxikodon störst «Soinikoski M, Kuusniemi K, Jalonen J ym. A nationa...»165 och dess farmakokinetik är känd även hos äldre patienter «Kokki M, Välitalo P, Rasanen I ym. Absorption of d...»166.
    • Dosen väljs enligt patientens ålder, vikt, allmäntillstånd, graden av smärta, effekten av tidigare doser och eventuella biverkningar.
      • Med åldern sker förändringar i opioidernas farmakokinetik och farmakodynamik som leder till att opioideffekten förstärks och varar längre.
      • Om patienten har medelsvår eller svår njursvikt, ska läkemedelsresponsen följas noggrant upp vid upprepad dosering, och vid behov ska engångsdosen reduceras på grund av risken för kumulering av oxikodon och dess metaboliter.
    • Oxikodon påbörjas i små doser på 1–2 mg intravenöst på akuten eller uppvakningsavdelningen.
    • Som alternativ till intravenös dosering kan oxikodonlösning ges buckalt (på munhålans slemhinna), oralt eller subkutant. Engångsdosen är initialt 2–5 mg «Kokki M, Välitalo P, Rasanen I ym. Absorption of d...»166.
    • När behandlingen fortsätter, doseras oxikodon som orallösning eller i tablettform.
    • Alltid då en höftfrakturpatient ordineras ett opioidanalgetikum, ska samtidigt ett laxativ ordineras. Alternativt kan ett opioidpreparat som också innehåller naloxon ordineras «Kuusniemi K, Zöllner J, Sjövall S ym. Prolonged-re...»167.
    • Användningen av opioidanalgetika är temporärt. Dosen minskas och medicineringen avslutas efter operationen, då smärtan lättar.
  • Det finns inga forskningsrön om användning av kodein och tramadol för behandling av smärta hos äldre höftoperationspatienter. Dessa preparat rekommenderas inte för behandling av akut smärta hos höftfrakturpatienter på grund av risken för biverkningar, risken som CYP2D6-beroende metabolism medför och risken för njursviktsrelaterad kumulering av metaboliter «Solomon DH, Rassen JA, Glynn RJ ym. Läkemedelsbehandling av smärta efter akuta fasen

  • Regelbunden smärtlindring under de första veckorna efter frakturen förefaller att påskynda rehabiliteringen av höftfrakturpatienter «Chin RP, Ho CH, Cheung LP. Scheduled analgesic reg...»153.
  • Det finns inga forskningsrön om vilka läkemedel som borde väljas för behandling av smärta under rehabiliteringen av höftfrakturpatienter.
  • Arbetsgruppen rekommenderar att man för smärtbehandlingen hos höftfrakturpatienter i första hand använder regelbundet ett oralt paracetamolpreparat (1 g x 2).
  • Eventuell användning av NSAID-preparat ska vara kortvarig för att undvika skador på hjärt- och cirkulationsorganen och matsmältningskanalen (God medicinsk praxis-rekommendationen Kipu «Kipu»6 (på finska); Smärta «Smärta»7 (på svenska)).
  • En del patienter behöver opioidianalgetika ännu efter den akuta fasen. Användningen av opioidianalgetika ska dock vara temporär.

Annan perioperativ vård

  • Antibiotikaprofylax med en engångsdos av cefalosporin av andra eller tredje generationen minskar risken för sår-, urinvägs- och luftvägsinfektioner hos patienter som behandlas operativt (osteosyntes, del- eller helprotes) «Gillespie WJ, Walenkamp GH. Antibiotic prophylaxis...»174.
  • Personalen ska ombesörja att patienten får tillräckligt med vätska och näring. Patientens näringsintag ska uppföljas och vid behov ska patienten få hjälp med att äta. Om matintaget inte lyckas, ska tillräckligt intag av vätska och näring säkerställas på andra sätt.
  • Störningar i tömningen av urinblåsan och katetriseringar av urinblåsan ökar förekomsten av urinvägsinfektioner. Se God medicinsk praxis-rekommendationen Virtsatieinfektiot «Virtsatieinfektiot»9 (på finska); Urinvägsinfektioner «Urinvägsinfektioner»10 (på svenska).
    • En inneliggande kateter ska avlägsnas så snart som möjligt efter operationen.
    • Risken för urinvägsinfektion kan minskas med tillräcklig vätsketillförsel och behandling av förstoppning.

Kirurgisk behandling av höftfraktur

Behandling av icke-dislokerad fraktur på lårbenshalsen

  • Behandlingsalternativen är delprotes och osteosyntes. Båda tillåter tidig mobilisering och ger patienten möjlighet att stöda på benet med hela vikten.
    • Osteosyntes lämpar sig för yngre, aktiva patienter som har god benkvalitet. För övriga patienter är delprotes det primära behandlingsalternativet.
    • Enligt en norsk registerundersökning förekom det färre reoperationer om delproteser hade insatts än om osteosyntes hade utförts. Patienter som behandlats med delprotes hade också lindrigare smärta än patienter som behandlats med osteosyntes. Dessutom var de som fått en delprotes nöjdare och hade bättre livskvalitet «Gjertsen JE, Fevang JM, Matre K ym. Clinical outco...»183. Skillnaden blev särskilt tydlig i de äldsta åldersgrupperna.
    • Den vanliga osteosyntesmetoden i Finland är kanylerad skruvfixering. Metoden med platta och glidskruv kan dock vara en mer stabil fixeringsmetod än kanylerad skruvfixering och metoden är inte förenad med ökad komplikationsrisk «Siavashi B, Aalirezaei A, Moosavi M ym. A comparat...»184.

Diagnostik

  • Behandlingen av osteoporos hos höftfrakturpatienter kan inledas utan mätning av bentätheten, förutsatt att cancer och andra orsaker till sekundär osteoporos uteslutits (God medicinsk praxis-rekommendationen Osteoporoosi «Osteoporoosi»2 (på finska); Osteoporos «Osteoporos»3 (på svenska)).
    • Det bör finnas möjligheter att konsultera en specialist i internmedicin eller geriatriker för diagnostiken av osteoporos.

    • I en undersökning som utredde effekten av zoledronsyra var dödligheten i behandlingsgruppen 4 procentenheter lägre än i jämförelsegruppen (NNT 27, den relativa riskminskningen 28 procent) under en uppföljningsperiod på i genomsnitt 1,9 år. Rehabilitering av höftfrakturpatienter

  • Omfattande och multiprofessionell rehabilitering kan förbättra patienters funktionsförmåga, främja deras hälsa och ge resurser för förbättrad livskompetens. Målet med rehabiliteringen av höftfrakturpatienter är att så snabbt som möjligt återställa funktionsförmågan så att patienten kan fortsätta sitt liv i sin tidigare livsmiljö.
